Responsibilities
- Will be working out of the Ann Arbor office and servicing consumers in Washtenaw, Monroe, and Livingston Counties
- Coordinate access to community resources for achieving independent living goals
- Provide coaching and peer support to aid individuals in effectively managing disabilities
- Educate nursing facility residents and staff about available services through Disability Network Washtenaw Monroe Livingston
- Facilitate person-centered planning for transitioning individuals
- Develop comprehensive transition plans including housing assessments, skill training, and service identification
- Act as a liaison with community agencies involved in the transition process
- Secure and manage direct transition funds for successful reintegration
- Offer follow-up support services to individuals’ post-transition
- Attend relevant training sessions to enhance service quality
- May be required to perform duties in outdoor environments
- All other duties as assigned
- Bachelor’s degree in social work, nursing, health or human service-related field, and or a Community Health Worker certificate (required)
- 3 years of experience in healthcare or care of individuals (required)
- Valid Michigan driver’s license and proof of vehicle insurance
- Personal experience with disability and/or knowledge of disability issues and the consumer/community-based IL philosophy
- Possesses strong communication skills
- Must be able to sit for extended periods and lift objects weighing up to 50 pounds
