General Description
Oversight of staff casework and residents in the Apartment Treatment Program. Facilitates the care and coordination of treatment and ensures quality of care is being provided to residents in accordance with agency philosophy, policies and procedures.
Duties and Responsibilities
- Provides adequate care and services to assist in the transition and integrate support services that encourage development of skills necessary to improve quality of life and outcomes for ATP residents.
- Coordinates the admission, movement, and discharge of residents within the Apartment Treatment Program.
- Conducts intake screenings as needed and makes recommendations regarding admission.
- Conducts clinical assessments and coordinates care and treatment in conjunction with program staff.
- Ensures health, psychiatric, medication and case management services are provided for each resident within the program. Activities may include referring and linking residents to services, advocating and monitoring delivery of services.
- Coordinates development and implementation of individual service plans, reviews, behavioral contracts, and discharge plans.
- Makes visits to residents as needed.
- Oversees coordination of residents’ skill development training.
- Assists residents in establishing vocational/educational goals and improving socialization skills.
- Coordinates and participates with residents in recreational activities.
- Provides crisis management services. Includes making collateral contacts.
- Ensures regular contact with all members of individual resident’s treatment team, including family members.
- Provides supportive counseling and ensures supportive counseling is offered to each resident.
- Ensures implementation of the Medication Management Training Program. Responsible for learning about medications and side effects.
- Meets regularly with individual staff to provide supervision and ensure quality care.
- Ensures appropriate apartment coverage.
- Coordinates and conducts weekly staff meetings.
- Ensures apartment meetings occur regularly- at a minimum of twice monthly or more often as needed.
- Ensures inter-agency communication takes place.
- Ensures maintenance, quality and confidentiality of all written materials in residents’ records.
- Maintains Petty Cash.
- Interviews potential staff members, and makes recommendations to the Program Director and HR regarding hiring. Human Resources reserves the right to make final decision regarding hiring.
- Evaluates performance of program staff.
- Informs Program Director and HR of personnel issues and recommends possible disciplinary action.
- Participates in and/or provides in-service training.
- Participates in agency and/or program development with other member of Administrative Staff.
- Meets regularly with Program Director for supervision.
- Communicates regularly with Program Director and Associate Director regarding residents and program specific issues.
- Ensures intra- agency communication.
- Provides on-call availability to assess and intervene in programmatic and/or residents’ medical or psychiatric emergencies.
- Participates in Utilization Review and Quality Record Audits.
- Ensures implementation of the Fire Safety Program.
- Ensures the coordination or transportation as needed for shopping, medical appointments, recreation activities, etc.
- Coordinates treatment team meetings.
- Ensures the proper notification of appropriate supervisory personnel of any medical, psychiatric, or programmatic emergency.
- Ensures the timely distribution of residents’ Money Management and personal needs allowance monies as indicated.
- Assures that resident’s fiscal records are maintained.
- Coordinates purchasing or program provisions and supplies with the Purchasing Coordinator.
- Conducts apartment inspections to determine what repairs are needed. Reports problems to appropriate personnel and follows through with submitting work orders as needed.
- Facilitates weekly staff meeting, weekly supervision, and other agency meetings.
- Attends work related workshops/conferences as program schedule permits.
- Participates in agency wide committees.
- Ensures completion of Medicaid Billing documentation.
- With approval from the Associate Director, negotiates renewal of apartment leases.
- Intercedes with landlords/ neighbors regarding specific issues involving residents.
- Responsible for completing all required documentation in a timely manner
- Learns and adheres to all agency policies and procedures on an ongoing basis.
- Accepts and follows through on any other task or duty as assigned or required.
Qualification and Competencies
- Bachelor’s Degree required in Human Services field. Master’s Degree preferred.
- Three (3) years’ experience working in a human service-related field.
- Supervisory experience not required but preferred.
- Ability to implement and manage change in a fast-paced growing business environment.
- Critical thinking and sound judgment skills
- Computer/office equipment usage
- Ability to work independently and as part of a team.
- Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families and the community.
Physical Demand and Working Conditions
- May be required to travel to various site locations in the field including other Agency sites or office locations.
- This position needs to occasionally move about inside the office and off-site locations to access files, file cabinets, office machinery etc.
- May provides transportation to clients in either personal vehicle or agency vehicle.
- May be responsible to physically assist clients move or transfer apartments.
- Assist and/or demonstrate cleaning techniques to clients to improve independent living skills.
- Lift charts/files weighing up to 20 pounds to and from various office locations
- Provide On-Call responsibilities after normal business hours, including weekends and/or holidays.
