Saint Alphonsus Medical Group (SAMG) is a physician led and accountable 650 provider multispecialty group practice seeking a Family Practitioner (MD / DO) to work full-time at its Lake Hazel clinic. The Family Medicine physicians at the Lake Hazel clinic are dedicated to providing services for a full spectrum of patients to include birth to end of life. The 8,000 square foot clinic is surrounded by a previously underserved residential area that continues to experience growth and development of new housing. The Lake Hazel clinic has procedure rooms, lab and digital X-Ray services on site in addition to a fully staffed Urgent Care facility open every day 8am – 8pm. The Lake Hazel clinic utilizes EPIC for Electronic Medical Record.
Support : 24 / 7 CRNA Anesthesia; Level IV Trauma Center; ED; Saint Alphonsus FM clinic on-site; community family medicine clinics; SAHS specialty physicians rotate in outreach, larger system resources.

Requirements :
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree
- Successful completion of an accredited ACGME / AOA Family Medicine residency program
- Board certification through the American Board of Family Medicine (ABFM) or American Osteopathic Board of Family Physicians (AOBFP) ( or obtain such certification within 3 years of employment )
- Obtain Idaho medical licensure to practice medicine in all its phases; valid controlled substance registration with Idaho Boards of Pharmacy & DEA
