Schedule: Monday - Friday | 8:00am - 5:00pm
Job Summary: As an interdisciplinary team member, works collaboratively with and provides administrative and clinical support to other treatment team members, patients and their families. Reviews medical records to facilitate communication of relevant patient related data to a variety of internal and external resources, as needed to achieve high quality care and effective patient outcomes.
Minimum Education: Bachelor's Degree in a health related field.
Licensure, Registration and/or Certification: None.
Work Experience: 6 months to 1 year of related experience.
Knowledge, Skills and Abilities: Working knowledge of Microsoft Word, Excel and Access as might be used in the preparation of correspondence and reports. Effective interpersonal, written and oral communication skills and efficiency, creativity, diplomacy and attention to detail accuracy. Ability to analyze data to discover facts, or develop knowledge, concepts or interpretations. Ability to organize and prioritize work in an effective and efficient manner. Ability to demonstrate efficiency, creativity, diplomacy and attention to detail and accuracy. Ability to provide excellent customer service working collaboratively and effectively with diverse people
Essential Functions and Responsibilities: Facilitates patient assessments and screenings. Participates in department admissions and discharge processes, as assigned. Coordinates and completes patient documentation, accurately and timely. Maintains referral source database, appropriate to assigned patient and department needs. Collaborates with team members; attends interdisciplinary/treatment team and patient planning meetings. Coordinate case management with utilization management.
Decision Making: Independent judgment in making decisions involving non-routine problems under general supervision.
Working Relationships: Coordinates activities of others (does not supervise). Leads others in same work performed (does not supervise). Works with internal customers via telephone or face to face interaction. Works with external customers via telephone or face to face interaction. Works directly with patients and/or customers. Works with other healthcare professionals and staff.
