Job Description
The Continuing Care Coordinator plays a critical role in supporting our residents throughout their treatment journey. Key responsibilities include:
- Providing ongoing case management and coordination of care
- Developing and implementing individualized discharge plans
- Fostering long-term relationships with referral sources
- Conducting on-site meetings and collaborating with multidisciplinary teams
Key Qualifications
- Bachelor's degree in Social Work, Case Management, Health and Human Services, or related field
- Two years' experience in residential or hospital settings working with adult psychiatric residents/clients
- Valid Driver's License and ability to obtain Driving Approval from SJVC
- Ability to travel locally and overnight when necessary, up to 40%
- Knowledge of and experience working with Clergy and Religious preferred
- Ability to maintain confidentiality and handle sensitive information according to HIPAA guidelines
- Exceptional communication and interpersonal skills, with a focus on teamwork and collaboration
- Commitment to respecting and supporting the values and principles of the Catholic Church
