Key Responsibilities
- Assist residents with ongoing case management, discharge planning, and aftercare services.
- Work closely with referral sources to build long-term working relationships.
- Coordinate treatment and aftercare services to ensure seamless transitions for residents.
- Maintain accurate records and reports to ensure compliance with regulations.
Requirements
- Bachelor's degree in Social Work, Case Management, or a related field.
- At least two years of experience in residential or hospital settings working with adult psychiatric residents/clients.
- Valid driver's license and ability to obtain driving approval from SJVC.
- Ability to travel locally and overnight when necessary.
- Knowledge of and experience working with clergy and religious preferred.
- Excellent communication and interpersonal skills.
