POSITION SUMMARY:
Early Intervention Services is the provision of a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum. The EIS Lead is a professional who collaborates with physicians, health care team members and patients to coordinate care for specific patient populations across the continuum. Provides various social work services to meet needs of patients including leading groups and mental health crisis interventions in addition to routine medical case management activities: biopsychosocial assessment, care plan, insurance navigation and linkage to other resources necessary for patient engagement in care. Provides mentorship, guidance, and support to other EIS case managers. Social work practices and services are consistent with applicable accreditation and licensing standards and reflect the National Association of Social Worker Code of Ethics.
