What are the responsibilities and job description for the Bilingual Community Health Worker position at Saint Peters Healthcare System?
The Community Health Worker is responsible for providing outreach and case management to clients; providing assistance to the delivery team (physicians, nursing staff, nutritionist, or social workers) by coordinating care and services for patients/clients in order to maximize use of available resources and to offer complete preventative care to those in need. Provides follow up phone calls and home visits to patients and their families to help teach and train in areas of health logistics, health behavior modification and other life and health skills to manage their health. Works under the supervision of a Nurse, Case Manager, nutritionist, DSRIP Project Manager, or Social Worker.
Qualifications:
• Health/Social Services experience or education
• Excellent verbal and written communication skills with focus on customer service
• Valid NJ driver’s license
• Case management experience preferred
• Strong computer skills
• Knowledge of cultural and lifestyle diversity issues with ability to network with a specific population
• Ability to work both independently and as part of a team; Strong organizational, case management and interpersonal skills
• Bilingual
