What are the responsibilities and job description for the Social Worker II position at Samaritan Health?
Pay Range - $20.23 - $31.36
Provides psychosocial assessment, evaluation and care planning as well as coordination of appropriate resources to assist residents with adjustment to the facility. Acts as an advocate and helps others to understand the needs of each resident. Also coordinates discharge planning. Must be able to effectively work with the interdisciplinary team as well as provide residents with the highest practical level of physical, mental and psychological well-being and quality of life. Candidates must have the ability to communicate effectively with medical professionals internally as well as with outside agencies. He/she must possess outstanding interpersonal, non-judgmental and supportive skills as well as organizational and time management skills. General knowledge of Social Security, Medicare and Medicaid criteria and regulations. MDS knowledge helpful. Basic computer knowledge and understanding of medical terminology required.
· Bachelor’s degree in social work or human services field required. Prior work with the elderly and/or disabled population preferred. Candidates should have a working understanding of social systems along with the ability to implement state and federal regulations.
