About the Role:
We are seeking a dedicated PCA Direct Care professional to use independent judgment and discretion in providing direct service and case management services to persons with developmental disabilities who are receiving residential services through Sample Supports. This position provides skilled ongoing direct care services to clients living in the supported apartment program while accessing various community settings. Supports will be based on the needs of the clients, as well as skill acquisition as outlined in their individualized plan.
Clients receiving services in this program generally have extensive mental health needs and/or substance abuse in addition to developmental disabilities. This position also requires extensive case management while having the ability to transition often between direct contact with clients, and basic case management including apartment visits, medical appointments, advocating for clients in a courtroom setting, etc.
Key Responsibilities:
- Provide direct client services to individuals within the PCA program, working collaboratively with Residential Coordinators, to ensure residential goals are obtained
- Transporting and assisting clients in accessing various community settings including, but not limited to, grocery shopping, medical appointments, courtroom settings, etc
- Complete daily tracking and necessary documentation to ensure compliance and company standards are met
- Assist in the completion of various assessments, pre-IP documents, medical documentation, financial documentation and forms, as needed
- Routine home visits to assist the clients in the cleaning of their apartments and check on their overall wellbeing.
- Provide PCA clients with day program services and arrange weekly activities such as BINGO, craft projects, meal prep and cooking, and community outings
- Provide direct care services to family caregivers as needed
- Assist in implementation of individualized life skills curriculum
- Maintain a positive attitude and calm demeanor during difficult situations, modeling effective crisis management and problem solving for others
- Model appropriate skills and communication when working with clients with developmental disabilities and their teams
- Effectively support current clients to ensure a positive quality of life and community integration
- Maintain positive rapport with clients, guardians, and team members to ensure clients maintain their services with Sample Supports
- Advocate for client needs and appropriate services to ensure clients build skills and work toward residential goals
- Ensure that all client paperwork meets expectations for successful billing and potential claims testing
- Maintain successful communication with all team members and establish a comprehensive case management approach, with the agency as a primary member
- Assist in overall residential program development
- Assist coordinators in maintain positive relationships with housing partners and actively recruit new housing partners for future client needs
- Attend internal and external meetings as needed
- Reliable transportation is required to get to work/drive clients
- Any other tasks as assigned

Requirements
The ideal candidate possesses the following characteristics/skills:
- Must be 18 years of age or older due to demands of driving a vehicle on behalf of the organization
- High school diploma or GED required, and 1 year of experience in related field
- Must meet approved motor vehicle record standards for providing transportation for clients
- Must be able and willing to provide physical intervention to clients as needed, implementing NVCI techniques
- Must have training and/or experience commensurate with the service or support being provided
- Works in full compliance with all of Sample Supports' policies, procedures and protocols Job responsibilities include reporting any suspected maltreatment, abuse, neglect, or exploitation of an at-risk individual. (M/A/N/E) Each position is considered a mandatory reporter
- Reliable transportation is required to get to work/drive clients
- Able to perform the physical demands of the position
Physical Demands:
Due to the flexible nature of positions and expectations in a team environment, all employees are expected to fulfill the physical demands of each position. Employees are expected to be available to one another, cover shifts, and work together to get the job done.
Lifting:
- Employees are expected to lift 75 pounds, depending on specific job site expectations.
Motion:
- Employees are expected to utilize a full range of motion and be able to stand and walk for the duration of their shift (8 hours).
- Employees are expected to physically intervene with clients that pose a safety threat to themselves or others utilizing NVCI-approved techniques.
- Employees may be expected to run/jog for 20-30 minutes if necessary in the event of a client elopement.
- Must be able to lean, squat, and bend in order to appropriately administer appropriate care to client, including administering first aid, and/or CPR for up to 45 minutes.
- Must be able to assist clients with gait issues, offering physical assistance and support in all weather conditions.
- Must be able to lift and/or assist clients in and out of vehicles.
- Ability to be able to work in front of a screen (computer, phone, etc.) for 6-8 hours a day.
- Employees are expected to physically intervene with clients that pose a safety threat to themselves or others utilizing NVCI-approved techniques.
Working conditions
- Sample Supports' business hours are 24 hours a day, 7 days a week so employees should expect to work some evening and weekend shifts
- Ability to work independently, meet deadlines with minimal supervision, schedule time efficiently
- Must be comfortable working with and around individuals who may exhibit unpredictable and/or volatile behaviors
