DESCRIPTION OF WORK: The Sampson County Health Department provides medical services to targeted populations in the Sampson County area to provide quality healthcare, promote a safe environment, and partner with the residents to foster healthy lifestyles and the importance of prevention through screening, education, referral, and follow-up. The primary purpose of this position is to provide case management services for families who have children that qualify for the North Carolina Care Coordination for Children Program (CMARC). This position also provides care managements services for pregnant women who qualify for the North Carolina Pregnancy Care Management Program (CMHRP).
KNOWLEDGE, SKILLS & ABILITIES: This position requires considerable knowledge of social work principals, techniques, and practices and their application to individual casework, group work, and community problems ; knowledge of psychosocial , socioeconomic, and behavioral problems and their treatment ; knowledge of governmental and private organizations and resources in the community knowledge of laws, regulations and policies which govern the program; general knowledge of medical terminology, disease processes and their treatment ; and knowledge of principals of physical growth and development, especially as they relate to pregnancy. Position requires accuracy in work due to assessment, treatment, and follow-up that directly impacts the client's well-being. Must be able to work independently daily with general instructions provided by Supervisor. Position will require close contact with clients, families, nurses, physicians, and other health care and human service professionals throughout the community to provide case management, counseling, and follow-up services. Individual must be able to establish rapport with clients, human service contacts, and healthcare professionals.
MINIMUM EDUCATION AND EXPERIENCE: Minimum of a bachelor's degree in social work (BSW, BA in SW, or BS in SW) from an accredited school of social work; bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
