DUTIES AND RESPONSIBILITIES
Functions as part of a multidisciplinary team consisting of social workers, nurses and
administrative staff
Provides case management, case assistance and advocacy for older adults in the office, in their
homes and via telephone.
Assists seniors in identifying and applying for benefits and entitlements for which they may
qualify
Documents all client contacts in the NYC Aging’s Peer Place application in a timely manner
Meets with potential new members to explain services available
Facilitates support groups, educational/recreational activities and intergenerational programming
to meet contractual obligations and the needs of Deepdale Gardens residents over the age of 60
Collaborates with program team to create new programming, special events, calendars, flyers,
and program guide descriptions for activities
Interfaces with NORC Advisory Board members, Program Officers from NYC Aging and NYS Office
for the Aging
Other duties as assigned
EDUCATION, EXPERIENCE, AND REQUIRED COMPETENCIES
Licensed Master’s Degree in Social Work and experience working with a geriatric population
required; if not yet licensed, must become licensed within 6 months of employment
Knowledgeable about benefits and entitlements relative to older adults
Experience utilizing Google Docs/Sheets, Microsoft Office applications, internet and email
Experience utilizing Zoom technology for virtual programming
SPECIFICATIONS FOR PHYSICAL REQUIREMENTS
Ability to travel to multiple service sites within the agency, and to make home visits
JOB DETAILS
- Schedule: Full-time, Monday through Friday, 40 hours; occasional evening or weekend hours may
be required
- Clinical hours towards “C” licensure and supervision available
- A car and valid driver’s license is required
- This is a Union position
- Salary Range: $62,250 to $67,500 depending on licensure
