Program Summary:
TransLink program is an innovative, peer-focused initiative designed to significantly reduce the risk of substance use, HIV infection, STI and viral hepatitis infections among homeless and unstably housed Transgender women living in San Francisco, CA.
Role Summary:
Program Specialist is responsible for client outreach, engagement, and navigation services for new and existing clients in the program and for maintaining and keeping confidential records on clients and providing support with client workshops. Program Specialist supports and reports to Program Manager of TransLink.
Essential Functions:
- Conduct a comprehensive client assessment, in collaboration with client, to provide linkage to resources and assist clients with access to care.
- Conduct consistent client follow-up to assess client progress and keep clients engaged in the program.
- Offer comprehensive navigation services, including helping clients keep scheduled appointments by accompanying them and advocating on their behalf during visits.
- Complete timely documentation of assignments and any other required documentation including, but not limited to, client interaction notes, responses to emails, presentations, etc.
- Conduct outreach activities to connect and enroll new clients to TransLink, as needed.
- Provide education on harm reduction, HIV, STI, and viral Hepatitis infections, helping clients gain the tools they need to maintain their health and well-being.
- Work closely with the TransLink team, including Peer Ambassador, to achieve program goals and provide clients with complete support and guidance.
- Help support the various programs and projects run by the Community Engagement team and the agency.
- Lead client workshops and support groups, collaborating with the Program Manager to maintain consistent client participation.
- Participate in staff meetings, trainings, and supervisions.
- Other duties as assigned
Minimum Qualifications:
- At least 1 year experience of working or interpersonal experience with transgender individuals and issues.
- At least six months experience with harm reduction, client-centered counseling, and motivational interviewing.
- Familiarity with issues and concerns of Transgender individuals.
- Ability to handle multiple tasks and to work professionally and ethically within multi-racial and multicultural settings.
- Ability to provide customer service in an effective, courteous, and friendly manner and meet the needs of diverse clientele.
- Ability to maintain confidentiality and privacy of persons, documents, data, and communications.
- Excellent written and verbal communications skills in English.
- Up to date with vaccinations, including latest COVID-19 vaccination and seasonal flu shot vaccine.
- Proof of TB clearance (issued within the past 12 months).
