San Luis Valley Health in Alamosa, Colorado is seeking a highly motivated, a full-time and part-time Behavioral Health Provider to provide behavioral health therapy services in our Alamosa and La Jara clinics. Provider will work closely with medical team to support patient outcomes, through screening, brief intervention, and counseling services. Successful candidates will have a strong desire to work with a medical team, provide consultation and expertise on behavioral interventions, and enjoy a fast-paced setting, implementing cutting-edge interventions. State licensure in social work, counseling, or psychology required. Position offers continuing education and competitive salary and benefits.
What You Need :
- Licensure required; Master's degree preferred.
- 2 years' experience post-licensure preferred.
- Experience working within medical setting preferred.
- Must be licensed in the State of Colorado.
- Must be credentialed or eligible to participate in State and Federal healthcare programs and private insurances.
- Must have current CPR certification.
- Knowledge of acute and chronic care protocols and in the delivery of behavioral health care.
- Knowledge of how to assess patient health.
- Knowledge of health promotion principles, techniques and patient learning measurements.
What You'll Do :
Provide direct patient care.Completes evaluations with variety of diagnostic tools, following best practices.Provides acute and chronic care according to behavioral health principles and practices.Monitors effective therapy.Knowledge of the development and maintenance of information / referral systems.Triage patient phone calls and evaluates patient problems.Provides triage and screening services for patients within clinic setting.Responds to medical emergencies including CPR.Communicates effectively with colleagues, patients, physicians and administration.Demonstrates positive and effective verbal and written communication skills with patients, families and co-workers, and in patient documentation.Educates patients and families.Provides staff with education formally and informally.Demonstrates efficient management of time and resources.Utilizes and conducts research / inquiry appropriate to clinical setting.Maintains a current level of clinical practice by selecting appropriate avenues for professional growth and routinely seeks continuing education according to best practice. Evaluates goal attainment and revises goals on an on-going basis.Adheres to and enforces SLV Health Administrative and Departmental policies and procedures.Performs all other duties as assigned.We Take Care of Our People
