San Luis Valley Health is seeking a Resource Coordinator Trainer to work collaboratively with providers and clinical staff to support patients with chronic conditions and/or complex needs according to the guidelines established by the oncology/hematology clinic. To facilitate communication, coordinate services, address barriers, and promote optimal allocation of resources while balancing clinical quality and cost management. The Navigator will work in the outpatient clinic setting alongside the Nurse Navigator and clinical team. This position will screen, assess, plan, and facilitate care using established care management standards and guidelines for chronic disease.
What You'll Need:
- Bachelors degree or comparable work experience required.
- Customer service skills
- Excellent oral and written skills.
What You'll Do:
- Provide at the shoulder training and support to staff working on grants, patient navigation, and case management
- Demonstrating sensitivity and responsiveness to a variety of cultural values and beliefs and social determinants of health
- Following all SLV Health policies and procedures
- Conducting basic screenings of patients within the oncology hematology clinic, including review of barriers to health self-management, depression, stress, and substance abuse.
- Providing basic crisis intervention services to patients in need and performing distress screening to assure patients are having follow up appointments if necessary
- Providing basic case management support regarding health and mental health needs, functional abilities, readiness to change, caregiver stability, social supports, financial resources, environment and safety concerns, life care planning and self-management skills
- Performing psychosocial coordination to assist patients with financial transportation, applications, patient assistance, or drug free programs
- Obtaining prior authorizations for oral and intravenous chemotherapy regimes, funding coordination, pharmacy coordination and assisting with other authorizations if needed.
