Discharge Planner/Social Services will be responsible for identifying and providing for each resident’s social, emotional, and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and assisting in planning his/her discharge. This position will directly report to the Social Services Director.

Discharge Planner/Social Services will be responsible for identifying and providing for each resident’s social, emotional, and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and assisting in planning his/her discharge. This position will directly report to the Social Services Director.
- Assist residents in obtaining ancillary services such as vision, dental, communication devices, and podiatry needs.
- Assist residents and families with adjustment to changes, including room changes. Assist families with the engraving and/or marking of residents’ personal items.
- Review residents’ rights with residents and staff, and attend to various personal needs of the residents. Assist in the development of family group meetings.
- Document progress notes, which relate to each resident’s discharge plan.
- Two (2) years of experience in social work is preferred
- Healthcare experience, especially in long-term care, is strongly preferred.
- Bachelor's Degree in Social Work. (SSW - Preferred)
- Basic understanding of computer technology.
- Must have the ability to read, write and follow oral and written directions in English.
- Must be able to relate favorably to residents and families and to work cooperatively with other employees and personnel of other community agencies.
- Will assist with HR duties.
- Must have knowledge of State and Federal guidelines.
- Must have demonstrated skills in communication, documentation, assessment, and social work methods techniques.
- Must be able to pass a drug screen and criminal background check.
