Responsibilities :
Discharge Planner / Social Services will be responsible for identifying and providing for each resident’s social, emotional, and psychological needs, and the continuing development of the resident’s full potential during his / her stay at the facility and assisting in planning his / her discharge. This position will directly report to the Social Services Director.
Assist residents in obtaining ancillary services such as vision, dental, communication devices, and podiatry needs.
Assist residents and families with adjustment to changes, including room changes. Assist families with the engraving and / or marking of
residents’ personal items.
Review residents’ rights with residents and staff, and attend to various personal needs of the residents. Assist in the development of
family group meetings.
Document progress notes, which relate to each resident’s discharge plan.
Qualifications :
Two (2) years of experience in social work is preferred
Healthcare experience, especially in long-term care, is strongly preferred.
Bachelor's Degree in Social Work. (SSW - Preferred)
Basic understanding of computer technology.
Must have the ability to read, write and follow oral and written directions in English.
Must be able to relate favorably to residents and families and to work cooperatively with other employees and personnel of other community agencies.
Will assist with HR duties.
Must have knowledge of State and Federal guidelines.
Must have demonstrated skills in communication, documentation, assessment, and social work methods techniques.
Must be able to pass a drug screen and criminal background check.
