Under general direction of the Youth Services Administrative Program Manager, provides intensive case management to assist youth and adolescents and their families in accessing behavioral health services.
EXAMPLE OF DUTIES
Assess the behavioral health needs of youth or family members, identify risk factors, and develop and implement a care plan that outlines goals and objectives.Provide support and guidance to youth and families during times of transition or crisis, specifically behavioral health concerns. Coordinate services with schools, hospitals, youth protective services agencies, mental health centers, homeless shelters, local behavioral health providers, and other entities to meet the youth’s needs.Provide youth and family members with a warm handoff to community-based services provided by other professionals such as therapists, psychiatrists, teachers, and any other system providing a service to the client.Serve as an advocate for youth and their associated caregivers and/or family members in accessing and maintaining adequate services.Monitor progress on goals and objectives, communicate with parties involved in the case, and make adjustments as needed.Communicate regularly with youth or their parents about their progress in meeting goals and objectives.Review reports from other professionals involved in the case, such as therapists or teachers, to ensure progress.Use the Unite Us Platform and other referrals to appropriate community resources.Work with youth and family through their care plans and collaboratively work with family to decide on appropriate discharge.Develop and distribute youth-specific behavioral health promotional materials. Establish and maintain regular contact with behavioral health providers and professionals in the community, including other governmental entities.Work cooperatively with other Divisions of the Community Services Department to maximize available resources.Knowledge/Skills: Communication skills: Specifically, the ability to effectively communicate with clients, other case managers, behavioral health agencies, medical facilities, and other stakeholders that play a role in a client’s plan. Communicate effectively, orally and in writing, and maintain the highest confidentiality, integrity, and ethical standards.Problem-solving skills: Assisting clients to develop solutions or plans to meet their needs and concerns.Organizational skills: Managing various tasks, multiple client files, and prioritizing information and tasks. Critical thinking skills: Analyzing a situation and deciding on a plan based on the information provided. Ability to cultivate and maintain effective working relationships with various community members, organizations, and agencies. Administrative skills: Knowledge of word processing, spreadsheets, databases, and other software programs in a Windows environment. Able to develop, organize, and implement programs and policies.
SUPPLEMENTAL INFORMATION
Working Conditions: Work is performed primarily in an office setting and in various community settings, including outdoors. Work schedule may include evening, weekend, and holiday hours. May be required to travel. While performing the duties of this job, the employee regularly is required to stand, walk, and sit; use hands to finger, handle or feel; reach with hands or arms; and talk or hear. Clarity of vision at short distance is required. May be required to lift up to 20 pounds. May be exposed occasionally to dust, fumes, airborne particles, or allergens; and to high risk or potentially dangerous situations. Will use protective clothing, equipment, devices, or materials occasionally. May be subject to exposure to CRTs and VDTs. Conditions of Employment: Selected candidate must submit to and pass a County paid pre-employment physical and drug/alcohol screening. Additionally, selected candidate must submit to and pass a county paid criminal background screening. Selected candidate must possess and maintain a valid New Mexico Class D Driver’s License as incumbent shall be appointed to drive a County vehicle during the performance of his/her duties.
