What are the responsibilities and job description for the LVN QA position at Santa Fe Home Health Care Inc.?
Scope of Position:
1. To ensure quality and safe delivery of home health care.
2. To implement and evaluate agency standards in all areas of documentation.
3. To implement and evaluate activities that reflects the agency’s philosophy and standards of care.
4. To ensure agency’s compliance with state and federal guidelines in documentation and coverage determinations.
5. To ensure quality and safe delivery of nursing care and therapy services.
Qualifications:
1. Licensed Vocational Nurse (LVN) licensed in the state of Texas or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
2. Understanding of Federal, State and local guidelines pertaining to home health agencies.
3. Understanding of Medicare guidelines and a general knowledge of the principles of billing and reimbursement.
4. Excellent verbal and written communications skills essential.
5. Excellent organizational skills required.
6. Proficient in the use of computer for data entry and file maintenance.
7. Attendance Requirements: 8:00 – 5:00 Monday – Friday; hours may vary depending on census, number of visits, staffing, etc.
Responsibilities:
1. Reviews clinical records for completeness daily to ensure all documentation has been obtained to support claim for payment.
2. Reviews CMS Forms (485-487) for completeness.
3. Audits all admissions, open and closed charts for completeness.
4. Completes clinical review audit sheets to supervisor regarding note deficiencies and corrections.
5. Demonstrates a working knowledge of Medicare guidelines for home health coverage and compliance.
6. Assists the medical clerk in filing and other related duties.
7. Assists with typing duties related to completing telephone orders.
8. Sorts, alphabetizes and files yellow copy of nursing notes in QA chart as needed.
9. Re-audits charts on an on-going basis for telephone orders returned by MD.
10. Pulls visit logs from computer to audit against nursing, home health aide and other disciplines for billing accuracy.
11. Audits all discharged records for completeness.
12. Any other duty as assigned by supervisor.

Pay: $20.00 - $24.00 per hour
