Job Summary
Under the supervision of the Office Supervisor and at the direction of the physician, the Assessment Referral Coordinator schedules appointments for continued care and provides necessary information for these appointmentsin collaboration with physicians and patients to ensure patients receive high quality care and exemplary customer service.
Essential Functions
- Ensures that patient chart entries are made accurately and in a timely matter and forwards charts as appropriate. Maintains patient files, records, and other relevant information.
- Assists with the scheduling of tests and treatments. Arranges referrals to a specialist and provides necessary authorizations
- Provides exemplary customer service to patients
- Provides clear and concise instructions to patients and families regarding referrals and follow-up treatments/appointments as necessary.
- Responds to and screens telephone calls for referrals.
- Performs follow up on referrals to obtain appointment dates and consults notes in a timely manner.
- Obtains authorizations/referrals in a timely manner and submits in ATHENA.
- Maintains and submits medical records request in a timely manner
- Performs other duties as assigned.
- Complies with all policies and standards.
Qualifications
- H.S. Diploma or GED required
- 2-4 years experience in a hospital or clinic setting preferred
Knowledge, Skills and Abilities
- Efficiently performing administrative duties.
- Scheduling appointments for continued care.
- Providing necessary information for appointments, including medical records and insurance referrals.
- Collaborating with physicians and patients to ensure high-quality care.
- Documenting thoroughly in medical records.
- Flexibility to perform additional tasks as assigned.
- Adapting to schedule changes and working overtime as required.
- Strong communication skills for interacting with patients and healthcare providers.
- Attention to detail for accurate scheduling and documentation.
- Strong phone customer service skills.
Licenses and Certifications
- CHAA - Certified Healthcare Access Associate preferred
