OVERVIEW:
JOB SUMMARY:
Working as a Caregiver in our Skilled Nursing Facility, you will assist residents with daily activities, promoting independence, and ensuring their safety and well-being, while also providing companionship. Caregivers work closely with the resident, their family, and the care team to provide personalized care that meets each individual unique needs and preferences.
RESPONSIBILITIES:
- Monitor and report any changes in the residents physical or mental health Assists with serving resident meals.
- Maintain resident confidentiality.
- Provide companionship and build positive relationships with clients to reduce feelings of isolation or loneliness.
- Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports.
- Encourage clients to participate in activities that promote physical, mental, and emotional well-being.
- Support clients in maintaining independence in their daily routines and decision-making.
- Ensure a safe environment by identifying and addressing potential hazards in community setting.
- Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.
- Collaborate with the care team.
- Communicate effectively with residents, families and team members.
- Present for staff meetings and trainings.
- Other duties as assigned
REQUIREMENTS:
- High school diploma or equivalent required.
- Previous experience in caregiving, home care, or community-based care is preferred.
- Ability to work independently and as part of a team.
- Patience, empathy, and a genuine desire to help others.
- Good organizational and time-management skills.
- Ability to perform physical tasks such as lifting, bending, and assisting clients with mobility.
- Must be able to stand, walk, and reach for extended periods.
- Ability to pass a background check
