- Medical Assistant - Clinical Care Coordinator I (Salary Range $20.00/hr-$27.65/hr)
- LPN - Clinical Care Coordinator II (Salary Range: $24.50/hr-$35.75/hr)
- RN - Clinical Care Coordinator III (Salary Range: $29.10/hr-$43.65/hr)
- Front Desk - Patient Relations Rep II (Salary Range: $18.00/hr-$25.50/hr)
*Pay rates are based on years of experience (and equity with others on the team).
Primary care patient population and duties: patients aged 18 , chronic disease management, annual wellness visits, point of care testing and vaccinations, coordination with specialty care offices for referrals and outpatient testing, handling daily patient calls, questions and prescriptions.
Pediatric patient population and duties: newborn to 17 years, annual wellness visits, point of care testing and vaccinations, developmental milestone assessments, handling daily patient calls, questions, and prescriptions, working collaboratively with care team, coordinating with pediatric specialty care facilities.

Required Qualifications
Clinical Care Coordinator I - Medical Assistant:
Requires current certification as a Certified Medical Assistant (CMA) by:
-American Association of Medical Assistants (AAMA),
-National Healthcare Association (NHA)
-National Center for Competency Testing (NCCT)
OR Registered Medical Assistant (RMA) by:
-American Medical Technologists (AMT)
-Nationally Registered Certified Medical Assistant (NRCMA)
-National Association for Health Professionals (NAHP); OR a certified EMT.
Note: Experienced medical assistants with a minimum of one (1) to two (2) years of experience, and with a certification from a certifying agency not noted above, or without certification, agrees to register with one of the currently accepted agencies for MA Certification and schedule a test date within 6 months from date of hire.
Test for certification may be beyond the six (6) month date.
Certification must be received and presented to HR within 12 months of date of hire.
- Requires Basic Life Support/CPR certification; or must be obtained within thirty (30) days of hire/transfer date.
Clinical Care Coordinator II - LPN:
- Require Basic Life Support/CPR certification; OR must be obtained within thirty (30) days of hire/transfer date.- Require graduate of an accredited school of nursing or a diploma program.
- LPN License (FL)
Clinical Care Coordinator III - RN:
- Require Basic Life Support/CPR certification; OR must be obtained within thirty (30) days of hire/transfer date.- Require graduate of an accredited school of nursing or a diploma program.- Require a minimum of one (1) year of clinical practice experience in a medical/physician office setting.
- RN license (FL)
Patient Relations Rep II - Front Desk:
- Require computer, keyboard and data input skills.- Require pleasant and professional demeanor.- Require skill in establishing and maintaining effective working relationships.- Require demonstrated initiative and self-direction.- Require ability to perform multiple tasks simultaneously, while maintaining proficiency and efficiency.- Require dependability/reliability.
- High School Diploma or GED required
Work Days/Shift/Start Time
*All position sare Monday – Friday.
*Day shift.
