Inpatient Rehab Facility (IRF)
Job Summary
The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client’s care. The rehab case manager assesses the patients’ needs upon admission and assists with the coordination of interdisciplinary treatment plans based on CMG and estimated length of stay, and FIM goals. The rehab case manager monitors and updates the interdisciplinary care plans as appropriate. The Case Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient’s right to self-determination (ACMA 2013).
Required Qualifications
- Require basic working knowledge of commonly used computer applications.
- Require Bachelor of Science in Nursing degree and active State of Florida Registered Nurse licensure if position is filled by a registered nurse. If position is filled by a clinical social worker, a Master in Social Worker (MSW) or Master in other health-related field is required with appropriate State of Florida licensure.
