The Clinical Care Coordinator II plays a vital role in ensuring the delivery of high-quality patient care, working closely with healthcare providers and support staff to manage patient flow and coordinate clinical services.
Key Responsibilities:
- Provide patient care according to age-specific population needs, utilizing expertise and knowledge to deliver compassionate and personalized care.
- Assist in the management of patient care, working collaboratively with healthcare providers to ensure seamless transitions and optimal outcomes.
Requirements:
- Basic Life Support/CPR certification is required; obtain within 30 days of hire/transfer date.
- Graduate of an accredited school of nursing or diploma program is required.
Preferred Qualifications:
- One year of clinical experience in outpatient physician office practice is preferred.
- Experience with electronic practice management and electronic health records is preferred.
- Demonstrated knowledge of electronic practice management and electronic health records software, along with skills in Microsoft Office software programs (Word, Excel, Outlook, and PowerPoint), is preferred.
Licenses and Certifications:
