What are the responsibilities and job description for the Psychiatric Liaison Service / Addiction Coordinator position at Saratoga Hospital?
Summary of Position: This position is responsible for Substance Use Disorder (SUD) consultations and assessments in coordination with the Emergency Department, Care Management, and Hospitalists. The position proactively identifies emergency department and hospitalized patients in need of consultation for SUD and completes consults in conjunction with staff psychiatrists (when indicated) and other members of the service. This position also provides education to staff about SUD and stigma associated with SUD. Performs SUD evaluations in the ED for patients identified with SUD problems. May perform mental health assessments in the Emergency Department when required by volume. As a primary therapist and member of the treatment team, plans and implement therapeutic services for an inpatient psychiatric population.
Primary Job Responsibilities:
- Patient Assessment and Discharge Planning: Completes SUD assessments and consultations on hospitalized or emergency department patients based on patient need. Prioritizes evaluation of patients with opiate use disorder, pregnant women, IV drug-users, persons who have overdosed and/or reversed with Narcan, persons with multiple chronic illnesses, or multiple hospitalizations due to SUD. Determines the appropriate level of care and makes recommendations. Assists with dispositions such as coordination with the care management staff, and transfer to substance abuse treatment programs, or other behavioral health services.
- Treatment Planning and Care Coordination: Participates in the development of the plan of care as a member of the multidisciplinary treatment team including the primary medical or specialty service, and care management service. Assists the Emergency Department in developing care plans for SUD patients identified as difficult or high-utilizers of services.
- Coordinates with out-patient providers and family in the development of the treatment plan. Provides ongoing psychotherapy following initial consultation. Utilizes evidence-based practices to facilitate engagement in SUD services. Provides psychoeducation on SUD related topics to patient and their family. Assists in management of hospitalized patients in SUD withdrawal.
- Serves as a liaison between the Emergency Department providers and Care Management staff. This includes connecting the patient to Medication-Assisted Treatment (MAT) if MAT is initiated in the ED.
- Collaborates with community based peer services and addictions programs in the community to facilitate engagement in recovery.
Minimum Qualifications: LCSW or LMSW eligible for LCSW within one year. Two years of experience in a behavioral health setting completing comprehensive behavioral assessments including diagnosis. Must be trained in non-violent crisis intervention at the time of hire or within 6 months of hire date.
Salary Range: $35.10 - $57.63
Pay Grade: 35
