Overview
As a Qualified Professional, you will be responsible for providing comprehensive support and guidance to adolescents residing in a group home setting. Your role will involve promoting their overall well-being, assisting with daily living activities, implementing treatment plans, and facilitating their personal and emotional development. To provide behavior support services to children and adolescents with emotional and behavioral disturbances by engaging them in weekly skills training services as well as targeted case management when deemed appropriate. The qualified professional shall perform 70% of the time when children and adolescents are awake and present in the facility.
Responsibilities
- Serve as a primary point of contact and advocate for adolescents in the group home.
- Develop and implement individualized treatment plans based on the specific needs and goals of each resident.
- Provide direct supervision and support to adolescents, ensuring their safety, emotional well-being, and adherence to house rules and regulations.
- Collaborate with a multidisciplinary team, including therapists, social workers, and medical professionals, to ensure coordinated care and consistent implementation of treatment strategies.
- Assist residents in developing and enhancing their life skills, including education, vocational training, self-care, and independent living skills.
- Foster a supportive and nurturing environment by facilitating group activities, recreational outings, and therapeutic interventions.
- Conduct regular assessments and progress evaluations to monitor the effectiveness of treatment plans and make necessary adjustments.
- Maintain accurate and up-to-date documentation, including progress notes, incident reports, and discharge summaries.
- Communicate regularly with family members, guardians, and relevant stakeholders to provide updates on residents progress and address any concerns or issues.
- Stay informed about current best practices, regulations, and trends in adolescent mental health and utilize this knowledge to enhance the quality of care provided.
Qualifications
- Experience in direct support or personal care within a home or long-term care setting is preferred.
- Knowledge of social work principles and practices related to developmental disabilities.
- Familiarity with resident rights and advocacy within care environments.
- Ability to lift and assist residents as needed, ensuring their comfort and safety.
- Experience working with individuals who have developmental disabilities, including memory care needs.
- Certification in first aid is a plus; willingness to obtain certification if not already held.
- Strong communication skills, both verbal and written, with an emphasis on empathy and understanding.
- A commitment to fostering a supportive, respectful, and inclusive environment for all residents.
Duties:
Provide clinical and administrative duties a minimum of 10 hours per week which includes individual
therapy in addition to administrative tasks including progress notes on clients.
