As a key member of our interdisciplinary team, you will be responsible for conducting long-term care MDS assessments and developing individualized care plans that prioritize resident-centered care.

Your Key Responsibilities:
- Conduct comprehensive MDS assessments to ensure accurate documentation and support Medicare/Medicaid reimbursement.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each resident.
- Stay up-to-date with regulatory changes and support compliance efforts to maintain the highest level of quality care.
- Maintain detailed records and participate in care planning meetings to ensure seamless communication and coordination among team members.
Requirements:
- Active RN license
- Minimum 2 years of experience as an MDS Coordinator
- Strong knowledge of MDS 3.0, PDPM, and Medicare/Medicaid regulations
- Detail-oriented and organized, with excellent communication and teamwork skills
