Scion Nonprofit Staffing has been engaged to conduct an immediate search for a Licensed Practical Nurse on behalf of our client, a community-focused organization dedicated to social justice, advocacy, and empowerment, particularly for marginalized communities in Seattle, Washington. This is a full-time, direct-hire, on-site opportunity with benefits!
Responsibilities:
Offer patients relational support in the form of health education, systems navigation, community resource identification and enrollment assistance.
Provide care coordination for patients.
Act as a liaison between community members and health, wellness, and social service resources.
Advocate for community members within local agencies.
Ensure compliance with data collection.
Help plan and facilitate community meetings, training sessions, and programs that help advance the health of community members and patients.
Develop and maintain confidential client notes and records.
Collaborate and cross-function internally and externally.
Specific responsibilities preferred include, but are not limited to:
o Blood draws and Lab sampling
o Obtaining samples for lab procedures
o Administering vaccines and other medications within scope of practice
Attend weekly in-person Team Meetings.
Qualifications:
Completion of a Certified program – with current/up-to-date licensure – as a LPN
Experience working in a healthcare setting or social support agency, preferably working with Black, Brown, Native, LGBTQ and otherwise marginalized clients with a focus on community-oriented care.
Prior experience working across the lifelong age spectrum is preferred.
Strong commitment to racial, social, and health justice and patient advocacy.
Salary : $83,200 - $93,600
