What are the responsibilities and job description for the Clinical Pharmacist Practitioner position at Scotland Memorial Hospital?
Practice within physician offices within Scotland Health Care System to provide protocol-driven, evidence-based patient care. Educates and consults with patients and providers regarding use of medications, disease management, potential drug interactions, and monitoring of drug therapies. Areas of focus may include, but are not limited to metabolic disease, cardiovascular disease, and anticoagulation. Team based care including working with multidisciplinary teams and care management may be involved.
