What are the responsibilities and job description for the CLINICAL PHARMACIST position at Southeastern Health?
Serves as a resource for, and evaluates appropriateness of drug therapy. Responsible for the accurate and timely preparation, manufacturing, compounding, repackaging, relabeling, dispensing and control of medications, intravenous fluids and other pharmaceuticals products.
- EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE :
- for practice specialty.
- Valid NC license to practice pharmacy.
- Incumbent will be required to attain a minimum of 20 hours of ACPE approved continuing education annually.
- ACLS for acute care pharmacists within 1 year of position, and certification maintained.
- Preferred :
- Speciality Sites (pharmacist has responsibility in area at least 30 hours / week).
- Completion of at least 2 approved certificate programs pertinentto specialty practice area. Certification by examination by anationally recognized body may be accepted as the equivalentof one program (i.e. ASCP Geriatric Certification). Secondprogram must be through completion of approved certificateprogram in practice area.
- Parttime Specialty Sites (pharmacist has responsibility in more than onearea, but no area at least 30 hours / week).
- Meets requirement in 2A or 2B for both practice areas.
- Clinical Pharmacist Practitioner approved by BOP and Medical Board for mid-level status plus one certificate program pertinent to practice area. Practice must involve delivery of collaborative care through CPP protocol for at least 30 hours per week.
Minimum Required :
BS Pharmacy or Pharm D, with Board of Pharmaceutical Specialty (BPS) certification, or equivalent
Above plus hospital experience, Pharmacy Practice or Specialty Residency.
BPS specialty certification appropriate for site (i.e.Oncology)
