Job Summary
We are actively seeking a Rehabilitation Counselor to play a key role in executing the rehabilitation services detailed in each client's Individualized Rehabilitation Plan (IRP). The Rehabilitation Counselor will work under the direct supervision of the Rehabilitation Specialist, ensuring that clients receive comprehensive and effective support tailored to their specific needs.
Key Responsibilities:
- Client Engagement & Service Implementation: Provide direct rehabilitation services to clients, following the goals and interventions outlined in their IRP.
- Intake Assessments: Conduct thorough assessments to evaluate clients' strengths, challenges, and rehabilitation needs.
- Individual and Family Support: Offer one-on-one and family-focused interventions to enhance coping skills, emotional well-being, and daily functioning.
- Group Rehabilitation Treatment: Facilitate structured rehabilitation groups designed to promote skill-building, social integration, and emotional regulation.
- Crisis Intervention: Respond to crisis situations promptly, providing immediate support and developing strategies to help clients stabilize.
- Rehabilitation Counselor are required to assess the needs of the client throughout their time in the program
- Completes all notes (direct and in-direct) on our EMR (electronic medical record software) daily.
- Collaborate with the Rehab Specialist and the client to complete treatment plan updates, including goals that were the focus of the services, the client's response to the services and a follow-up plan of action
- Attends team meetings and training as scheduled.
- Participates in supervision with the Rehabilitation Specialist.
- Collaborates with the client's assigned mental health therapists monthly and documents the type of collaboration (i.e. phone call, attending a meeting, etc.)
- Other duties as assigned
Additional Requirements:
- Use of own vehicle for visits to clients’ home
- *Must have experience working with adults and children
JOB REQUIREMENTS:
Basic Qualifications
- 1 year of work experience in a supervised mental health setting
- DLA Training Certification And other Relevant Behavioral Health Trainings

Pay: $25,000.00 - $57,836.00 per year
