The Housing Coordinator will work directly with clients to assess their housing needs, coordinate supportive services, develop housing plans, and collaborate with landlords and community partners. This role requires strong advocacy, case management, and problem-solving skills to ensure clients successfully obtain and sustain permanent housing.
Key Responsibilities:
1. Client Assessment and Engagement
- Conduct intake interviews and assessments to identify clients' housing needs, goals, and barriers.
- Develop and implement individualized housing stabilization service plans.
- Build trusting relationships with clients to provide guidance and support throughout the process.
2. Housing Support and Advocacy
- Assist clients in locating, securing, and maintaining affordable and stable housing.
- Provide education and support regarding tenant rights, responsibilities, and housing resources.
- Advocate for clients with landlords, housing authorities, and other stakeholders.
3. Case Management and Service Coordination
- Coordinate access to essential services, including financial assistance, employment support, mental health services, and substance use treatment.
- Monitor client progress and update service plans as needed.
- Maintain regular contact with clients to ensure continued housing stability.
4. Collaboration with Community Partners
- Build and maintain relationships with local housing agencies, shelters, and service providers.
- Stay informed about available housing programs and eligibility criteria in Minnesota.
- Act as a liaison between clients and community resources to address challenges and resolve conflicts.
5. Documentation and Compliance
- Maintain accurate, timely, and confidential records of client interactions, assessments, and progress.
- Ensure compliance with Housing Stabilization Services guidelines, policies, and state regulations.
Qualifications & Requirements
✅ Education & Experience:
- Bachelor’s degree in social work, human services, or a related field (preferred) or equivalent experience in housing support services.
- Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders.
- Knowledge of Minnesota housing resources, tenant rights, and supportive housing programs.
✅ Skills & Abilities:
- Strong case management, problem-solving, and crisis intervention skills.
- Ability to build relationships with diverse populations and advocate effectively for clients.
- Excellent written and verbal communication skills.
- Proficiency in maintaining detailed and organized client records.
✅ Other Requirements:
- Ability to pass a background check.
- Valid driver’s license and reliable transportation (preferred).
- Multilingual skills (Spanish, Somali, or Arabic) are a plus.
