Job Title: Resident Care Coordinator
Work Status: Exempt, Full Time
Work Hours: Tuesday-Saturday, 9:00 AM - 5:30 PM; may work evenings and/or weekends, as needed
Rate of Pay: $46,000-49,000 annually based on experience, plus quarterly and annual bonus incentive

Sedgebrook is looking for a hospitality-focused Resident Care Coordinator! This role is primarily responsible for conducting assessments and completing service agreements for residents who will receive care through our home care service, Sedgebrook at Home.

Here are a few of the daily responsibilities of a Resident Care Coordinator:
- Maintain knowledge of home care processes including scheduling, service provisions, and policy & procedure manual. Acquires and maintains sufficient knowledge of home care services to be able to assess quality of services, and to provide backup staffing support when needed.
- Conducts resident assessments and agreements for home care services, as needed. Participates in discussions and meetings with residents and their families regarding services needed to remain safe in their apartment home.
- Provides written communication and correspondence, maintains paperwork, files, etc. as it relates to home care.
- Supervises home care personnel in respect to scheduling, hiring, training, coaching, and corrective actions.
- Alternates taking on-call responsibilities.
- Performs specific work duties and responsibilities as assigned by supervisor.
Here are a few of the qualifications we need you to have:
- Minimum three (3) years' experience working with seniors, with a minimum of one (1) year experience working with seniors providing dementia level care.
- High school diploma or general education degree (GED) required.
- Strong customer service skills and passion for working with older adults.
- Knowledge of good safety practices in working with clients with physical limitations.
- CNA certification or LPN license preferred.
- Experience in caregiving or senior-related work preferred.
- Fluency in English, including speaking, reading, and writing.
