Coordinate the care of patients for the Complex Care Team. Assist providers and clinical staff in providing quality outcomes for Complex Care patients. Collaborates with Physicians, APP’s, staff & management for treatment of patients and assists with collection of patient data. Acts as coordinator or liaison with SRH and SMG specialists to improve continuity of care across health system. Assists with coverage of patient care when needed. Monitor and report out on defined metrics and compliance of care plans for patients. Responsible for educating clinical and nursing staff with education, treatment modalities and care of Complex Care services.
