Job Overview:
We are seeking a dedicated Case Manager to join our team. The Case Manager will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life.
Duties:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide support to clients in areas such as benefits, entitlement programs, housing, etc
- Supervise and monitor client progress towards established goals
- Coordinate with external agencies and service providers to ensure comprehensive care for clients
Skills:
- Strong background in social work, BA/BS required
- Proficiency in crisis intervention and management
- Knowledge of Burlington County resources and services
- Fluency in Spanish and Portuguese language skills preferred. but not mandatory
Schedule:
-Monday to Friday 9 am to 4 pm
-In person only, no remote
