Job Description
The Homestead at Anoka is looking for a Physical Therapist to work Saturdays or every other Saturday and/or on-call who is flexible, reliable, well organized and a team player to provide ongoing therapy evaluations and treatments and education to residents in a timely manner per physician's orders. To provide leadership and supervision of physical therapy services to other physical therapists, assistants and aides. The Physical Therapist evaluates and treats patients, communicates with families, physicians and other health team members and maintains appropriate documentation of services including MDS responsibilities complying with State and Federal regulations. Participates in on-going facility meetings and conducts in-service and training for facility staff on ongoing basis in restorative and maintenance techniques.
Qualifications
Must be a graduate from a physical therapy curriculum approved by the American Physical Therapy Association and is currently licensed as a Physical Therapist in state where practicing.
