Job Summary
Senior Community Services, the largest provider of services to Delaware County’s older residents and their families, is seeking Social Workers and Care Managers willing to provide supports and services to older adults. Specifically, services needed include ongoing care management, service coordination, and follow-up through home visits, family meetings, phone calls, and connection of consumers to community resources in order to help the consumer maintain the highest possible level of well-being so they can continue to live at home.
Responsibilities and Duties
The responsibilities of the Care Manager will include ongoing contact with assigned consumers, in-home reassessment of consumer needs on a regular basis, completion of the consumer care plan, revision of consumer care plan as needed, monitoring of service delivery, encouragement of family involvement, and advocacy on consumers’ behalf. Additionally, clerical and appropriate documentation of the aforementioned activities in an electronic consumer records database are two daily responsibilities.
Qualifications and Skills
Bachelor’s Degree which includes or is supplemented by 12 college-level credit hours in sociology, social work/social welfare, psychology, gerontology or other of the behavioral sciences; OR a Bachelor’s Degree in nursing; OR any equivalent combination of experience and training which adds up to four years of experience and education which includes:
12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other of the behavioral sciences. Qualifying experience would include coordinating assigned services as part of the client’s care or treatment plan, teaching client living skills, aiding in therapeutic activities, and providing socialization opportunities for clients.
It is preferred that the workers have some previous experience with the aging population and resources, but not necessary.
Strong verbal, written, and interpersonal communication skills are required. The candidate should also be computer proficient. A current driver’s license and insured vehicle to make home visits are mandatory, along with a Criminal Records check.
Physical Requirements include the ability to sit at a desk for extended times, look at computer screens for several hours, read small/ handwritten print, and hand-write/ sign documents. Additionally, workers will need to be able to lift a minimum of 25 lbs., walk up and down steps multiple times daily, use phones regularly and for extended times, and type.
Staff members are required to complete consumer visits in the community. The use of both a personal phone and secure home Wi-Fi is required for the position.
