Job Summary
The Social Work Case Manager is a critical part of the team at Senior Helpers making a differerence with our clients/family, caregiver and community! The position is part of a case management team.
Primary Responsibilities (including, but not limited to):
*Has a regular communication process for new clients that includes caregiver introductions, first day of service calls/intros, and a visit within the first 30 days of service, 60 (if needed), 90 day and change of condition reassessments.
*Ensures that clients and their families are satisfied with the Senior Helpers services, works to find solutions with the appropriate caregivers and/or family members.
*Visits clients on a regular basis to assess the quality of services as well as the client and family satisfaction and happiness level. Spends approximately 2-3 days out of the office doing assessments, re-assessing clients and managing client visits in the hospital, rehabs, facilities, etc.
*Develops and updates Client care plans and tasks.
*Updates and attends Client dashboard weekly.
*Communicates with Schedulers/HR/Recruiter needs.
*Reviews daily logs to ensure that each caregiver is following the care plan.
*Takes service inquiry calls and follows up with leads to meet revenue goals.
*Complete client assessments to determine what services are needed.
*Provide clients with community resources information (Hospice, home health, ADRC, etc.).
*Works with community agencies and care management groups to ensure the best care possible.
*Represents Senior Helpers at networking events and contributes to the marketing face of the company.
*Address all immediate policy violations with caregiver while in the field otherwise provide the HR Manager with written statement of policy violation, exclusion etc....
*Trains new caregivers during orientation on client care.
*On call rotation for urgent inquiries.
Qualifications
- Bachelor’s degree in Social Work
- Experience working with seniors preferred
- Must be able to drive, have a valid Driver's License and Car Insurance and pass driving check.
- Must be able to pass a state and country background check and drug screen.
- Excellent relationship management skills, demonstrating empathy and understanding towards clients' needs.
- Knowledge of relevant laws, regulations, and ethical standards in social work practice.
- Ability to manage multiple priorities while maintaining attention to detail.
- Strong communication skills, both verbal and written.
- May require after hours or weekend work depending on demands of client or prospect need.
