Senior Services, Inc. is seeking a Social Work Case Manager to join our dedicated Living-at-Home team in Forsyth County, NC. This role offers the opportunity to provide critical support through the Community Alternatives Program for Disabled Adults (CAP/DA), a Medicaid waiver program.
What You’ll Do:
As a Social Work Case Manager, you’ll collaborate with an assessment nurse as part of a multi-disciplinary care team to:
- Develop and manage personalized service plans for program participants.
- Monitor care to ensure quality and effectiveness, addressing psychosocial needs and barriers.
- Educate and empower participants and families in person-centered planning.
- Provide referrals, linkages, and ongoing support to community resources.
- Maintain meticulous records in compliance with CAP/DA policies and procedures.

Qualifications We’re Looking For:
- Education:
- Bachelor’s degree in Social Work with 1 years of community experience in health, medical, or homecare fields.
- OR a Bachelor’s degree in a related field with 2 years of similar experience.
- Skills and Abilities:
- Proficiency in Microsoft Office (Word, Excel) and adaptability to new software.
- Strong communication and interpersonal skills.
- Ability to handle sensitive information confidentially.
- Familiarity with care planning, community resources, and CAP/DA guidelines.
- Physical Requirements:
- Ability to perform sedentary work with occasional driving and use of technology.
