Geriatric Support Services Coordinator (Tuft’s and United HealthCare) Gloucester, MA
Full Time 35 Hours Per Week, Monday-Friday 9am-5pm
SeniorCare Inc. has a full time Geriatric Support Services Coordinator (GSSC) position opening. The Geriatric Support Services Coordinator integrates non-medical support services into individualized, community-based care plans and assists United Healthcare Senior Care Options (UHC SCO) and Tuft’s Senior Care Options (Tuft’s SCO) enrollees, their caregivers and Primary Care Physician in navigating the network of support and information services available to seniors in the community. The GSSC is an integral part of the Primary Care Team. The GSSC is responsible for all aspects of case management including needs assessments, service plan development and implementation as well as service authorization for specific services and monitors the service needs of consumers.
We are seeking an individual with a Bachelor’s degree in social work or related field or equivalent experience. Effective communication skills are necessary for interaction with the client, family and other supports. Must be able to utilize a computer for data entry, word processing and to research resources. Experience with elders and/or in a community service setting is preferred.
