Protective Services Intensive Case Manager

This is a part-time position, 30 hours per week.

SeniorCareInc. is seeking a Part-Time Protective Service Intensive Case Manager to support older adults with chronic psycho-social situations and heightened risks, including behavioral health needs. The PS ICM works to alleviate risks identified during investigations and assists clients in connecting with resources to improve their quality of life.

Responsibilities:

• Engage and support at-risk older adults in community settings
• Collaborate with clients and other involved parties to assess risk to older adults.
• Connect clients with available resources to address housing, safety, medical, and financial risks

Qualifications:

• Bachelor’s degree in social work or related field
• Minimum two years of experience with an elder population
• Skills in needs assessment, rapport building, task prioritization, and effective communication
• Knowledge of area resources
• Proficient in documentation and computer skills
• Available for on call rotations one week per month, from 7am to 11pm
