Sentara Community Care was founded with the goal of developing innovative care models that not only address but also help reduce health disparities in under-resourced communities. Since its inception in 2022, the program has grown rapidly in both scope and reach. We are seeking an experienced Physician to serve as an Associate Medical Director / Clinician across our Hampton Roads locations splitting time between Norfolk and Newport News, VA.
Position Highlights :
- Location : Newport News and Norfolk, VA
- 0.9 FTE Clinical, 0.1 FTE Associate Medical Director
- Experience treating family medicine, pediatrics, and adults with complex issues
- Shared call schedule, with RN first call
- Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients health holistically.
- Review quality and outcome metrics monthly with the healthcare team
- Collaborate with local civic and faith-based leaders on community education programs
- Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
