What are the responsibilities and job description for the Sentara Associate Medical Director for Community Care Clinics - Hampton Roads position at Sentara Health?
Sentara Community Care was founded with the goal of developing innovative care models that not only address but also help reduce health disparities in under-resourced communities. We are looking for an individual who is dedicated to addressing community needs, fostering relationships, and promoting overall health equity. This Physician will serve as an Associate Medical Director / Clinician across our Hampton Roads locations splitting time between Norfolk and Newport News, VA.
Position Highlights :
- Location : Newport News and Norfolk, VA
- Schedule : 2 days a week at each site, 1 day a week admin (Mileage reimbursement included)
- Experience as a licensed provider treating family medicine, pediatrics, and adults with complex issues
- Shared call schedule, with RN first call
- Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients health holistically.
- Review quality and outcome metrics monthly with the healthcare team
- Collaborate with local civic and faith-based leaders on community education programs
- Dedication to addressing community needs, fostering relationships, and promoting overall health equity.
