Summary
The Patient Care Coordinator is the point of contact to ensure patients' needs are met. The PCC coordinates patients' treatment plans and follow up, order changes, and renewals.
Essential Functions
- Determines insurance eligibility and coverage for a variety of plans and patient-specific situations
- Submits authorization requests to insurance in an accurate and timely manner
- Communicates with patients and families to assess supply needs
- Screens / rescreens patients for nutrition risk per established criteria
- Documents data collection results and makes appropriate referrals to the Dietitian as per
- Communicates effectively with physicians and dietitians regarding patient needs and required documentation
- Works directly with staff, patients, insurance companies and referral sources to ensure the best possible outcomes for our patients
- Documents communication and action plans thoroughly
- Troubleshoots product sourcing issues if applicable
- Special Projects as assigned
- Process customer orders, requests and information updates into a patient order management system
- Performs Order Changes and Renewals
