Job Type: Regular Scheduled Hours: 40 Job Summary: Reporting to the Operations Manager, the Targeted Case Manager is responsible for providing comprehensive case management services to individuals with a history of substance use disorder. They provide short-term continuity of care with the patient, patient families, and the treatment team. They build a formal and informal support network necessary to support long-term recovery and community integration. They assess the patient’s overall status, both substance use and behavioral health, and develop a plan for recovery in conjunction with the treatment team. In essence, a targeted case manager will provide the foundation to facilitate the long-term sustainability of an individual’s pathway to independence. Working with patients to build formal and informal support networks necessary to support long-term recovery and community integration. 2. Working with patients and their support network to reduce barriers to treatment linkage and provide psychological skill-building and motivational coaching to promote service engagement. 3. Participate in treatment team and make recommendations for patient status changes. 4. Maintain caseloads and utilization, by continuing regular contact with patients to monitor and assess progress. 5. Providing individual assistance to patients in accessing needed substance use and mental health services including accompanying individuals to appointments to address needs and assist with resource acquisition. 6. Providing individual assistance to individuals/families in accessing a variety of public services including financial and medical assistance and housing, including assistance on an emergency basis and directly helping to meet needs for food, shelter and clothing. 7. Interceding on behalf of individuals within the community-at-large to assist the person in achieving and maintaining their community adjustment. 8. Develop and maintain case and program documentation according to CMS and supervisory standards within electronic medical record. 9. Attend required meetings including individual and group supervision, treatment team meeting, and quarterly in-service. 10. Facilitate assistance with patient transportation to and from appointments. REQUIRED SKILLS AND KNOWLEDGE: 1. Maintain caseloads and productivity by ensuring regular contact with patients in community and office. 2. Monitor and assess progress per agency policy, regulatory requirements and best practices. 3. Meet with patients individually to assess needs; collaborate with patients, families and other targeted case managers and clinicians. 4. May provide, but is not limited to crisis intervention, advocacy, medication monitoring, transportation assistance, financial assistance, activities of daily living assistance, referral and linkage to community resources. May visit, monitor, and transport patients. 5. Develops and maintains a network of community referrals. 6. Prepare clinical reports and individualized service plans, utilizing agency electronic health record system. 7. Assure documentation is in compliance with contract, regulatory and agency requirements and best clinical practices. 8. Effectively work as a team member including participating in all required trainings, ensure the safety of others.
