Care Manager – South Sonoma County & Marin County Territory
Location: Serving Rohnert Park, Cotati, Petaluma, and Marin County
Salary: $65,650 annually
Schedule: Monday–Friday, 8:30 AM to 5:30 PM (with shared on-call duties after training)

What You’ll Do
As a Care Manager, you’ll be a key part of our team, serving as the link between clients, families, and caregivers to ensure exceptional care. You’ll be sharing the Sonoma County territory with another experienced Care Manager, with your focus on South Sonoma County (Rohnert Park, Cotati, Petaluma) and Marin County. Your responsibilities will include:
- Conducting client service inquiries, assessments, and routine in-home quality assurance visits
- Supervising, training, and evaluating caregivers—including surprise visits every two months
- Partnering with the Scheduling Department to match caregivers to client needs
- Following up with clients and caregivers to ensure satisfaction after care begins
- Assisting with staff training programs and attending regular team meetings
- Representing Sequoia at local events and within the community
On-Call Duties:
Following approximately 60 days of training, you’ll enter the rotating on-call schedule. On-call responsibilities are shared weekly among team members and include weekends. A company phone will be provided while on call.
Travel:
Up to 75% local travel within your designated territory; occasional travel outside the region may be required.
