Job Summary
The Client Care Coordinator ensures that all referrals are received in a timely, actuate detailed manner. They are expected to perform various duties related to client care, including client/company introductions and quality assurance visits with existing clients. Client Care Coordinators will continuously evaluate client care plans through a series of ongoing communications and visits to ensure high-quality care, client satisfaction and retention, and opportunities to increase service hours. They are liaisons between patients and the healthcare system, ensuring that patients receive the care they need and that they understand their medical condition, medication, and other instructions.
Primary Duties/Responsibilities
1. Receives all referrals and inquiries of programs that the agency offers as a source of service
2. Conducts client/caregiver introductions with every new client and with every new caregiver.
3. Completes and maintains client records, including all updates, schedule changes, and the necessary documented events, etc.
4. Staff/schedules all cases receiving care by the agency ( with appropriate supervision of the Nursing Supervisor)
5. Plan and execute a schedule that ensures each client has at least one quality assurance visit per month
6. Responds promptly and courteously to all client inquiries and issues.
7. Recognize and pursue opportunities to modify service plans to modify service plans to best support the ongoing needs of clients through a quality assurance program
8. Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues, and Caregivers to evaluate service.
9. Assists in reassessment and evaluations of field staff; audits on a weekly basis: caregiver’s time card and schedules for accuracy
10. Serves as a liaison between caregivers and the Nursing Supervisor.
11. Maintains regular attendance at all administrative staff meetings.
