We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that patients receive the necessary services and support throughout their treatment journey. You will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that promote optimal health outcomes. Having excellent communication, being able to work within a team and dependability are a must. This job requires RN Case Manager to visit patients at their residence. Looking for a team player. We are a supportive team looking to add a new member to our family.
**Responsibilities:**
- Manage and coordinate patient care throughout the healthcare continuum
- Timely documentation
- Conduct patient assessments and develop individualized care plans
- Collaborate with healthcare providers to ensure quality patient care
- Provide education to patients and families on treatment plans and disease management
**Experience:**
- Current Registered Nurse (RN) license in the state of practice
- Minimum of 2 years of experience in case management or a related field
- Strong patient assessment skills and ability to provide compassionate patient care
