Role Description
This is a full-time Care Manager Supervisor role located in Lakewood, NJ. The Care Manager Supervisor will be responsible for overseeing the quality of care management services provided, including case management, care coordinaroon, elder care, social services, and medicine. The Care Manager Supervisor will educate, train, and guide the work of care managers to ensure adherance to guidelines set by DOH and lead Health Homes for quality and compliance. The CM Supervisor audits the work of care managers and is also responsible for ensuring good-fit assignment of care managers to clients.
Qualifications
- Case Management and Social Services skills
- Elder Care and Medicine knowledge
- Experience in training care management staff
- Strong interpersonal and communication skills
- Ability to work well in a team and independently
- Bachelor's degree in Social Work, Nursing, Psychology, or related field
- masters degree preferred
