Join Our Team as an RN Supervisor for Home Care Services
RN SUPERVISOR for Home Care Services Responsibilities:
- Collaborate with the Patient Coordinator to ensure accurate clinical documentation for case openings.
- Initiate and revise the plan of care and medical orders.
- Conduct monthly client visits both in-home and virtually as required.
- Provide in-home supervision every 60 days or as necessary.
- Act as a liaison between Summit and the community, clients, facilities, etc.
- Evaluate the outcome of care and provide support to ensure that services meet the client’s needs.
- Inform physicians and other staff of changes in the client’s needs.
RN SUPERVISOR for Home Care Services Requirements:
- Current RN license in the state.
- 2 years of supervisory experience.
- Previous experience in Home Care.
- Exceptional staff training and customer service skills.
- Compassionate and friendly demeanor.
- Willing to travel up to an hour for visits
- Reliable transportation
